 *Must be a full time student (12 credit minimum)

  • Print out form
  • Submit form
  • We will call you to set up an appointment to validate the pass
    • Note: This pass will not be valid unless an appointment is set
  • Bring the printed, filled out pass to the appointment

Restrictions:

  • Must be 18 years of age or older
  • Must be a local resident
  • Valid only on your first visit
  • Please call for an appointment
  • Pass must be used in 3 consecutive days ONLY
